All forms must be complete and legible.

All regulatory charts must include the submitting
veterinarian's signature.

All regulatory charts must include complete
animal identification.

Samples delivered to the Serology Laboratory
must be sealed using the veterinarian's label or tape bearing
the veterinarian's signature.

Exports - the submitting veterinarian is responsible
for informing ADDL of any special requirements (i.e., dilutions,
type of test) using the ADDL form 3. If this information is
not supplied, the standard test will be used.

When calling for results, ADDL must have the
owner's name, the submitting veterinarian's Indiana license
number and bleed date exactly as written on the chart. ADDL
will not release results to owners.

Be aware that verification of any non-negative
samples will cause a delay in the availability of results.

Advance notice should be given when submitting
large groups.

If multiple tests are requested including Brucellosis,
the samples must be received 2 working days before the other
tests are scheduled to be set.

Samples for all other tests must be received
by noon the day before the test is scheduled to be set.

When requesting tests for Leptospirosis, all
species will be tested for these serotypes:

| L. pomona
L. grippotyphosa
L. hardjo
L. canicola
L. icterohemorrhagiae
|
In addition, swine will be tested for:

Tests performed at NVSL must be submitted through
ADDL.
